ABSTRACT

Abstract Introduction: Keloids and hypertrophic scars are dermal fibro-proliferative disorders unique to humans. Their treatment is a true challenge with multiple options, but not all the time with good results. Unfortunatelythis problem is not uncommon in patients with history of burn injury. Aim: To evaluate use of verapamil andpressure garments in patients with hypertrophic or keloid scar caused by burn injury. methods: We includedpatients with a hypertrophic or keloid scar caused by burn injury that report non-response to treatment withpressure garment. The pathologic scars were evaluated by serial photographic records, Vancouver and Posasscales. The scales of Vancouver and Posas were compared with t Student. Results: We included 13 scars in11 patients. Four scars were located in the legs, 4 in the arms, 4 in the face-neck and 1 in the abdomen. Thedose of verapamil was calculated 0.03 mg per kg. Injections were scheduled every 7 to 10 days until complete 6 sessions. Taking in count Posas scale, patients referred improvement in pigmentation (0.01), thickness(0.005), pliability (0.01), pruritus (0.003) and irregular surface (0.004). In the Vancouver scale the observers mentioned improvement in elevation (0.008), pigmentation (0.014), vascularity (0.022) and flexibility (0.014).No adverse effects were found in verapamil injection. Conclusion: Verapamil was useful in conjunction withpressure garment to improve the condition of the keloid and hypertrophic scar caused by burn.


Resumen Introducción: La cicatriz queloide o la cicatriz hipertrófica son desórdenes fibro-proliferativos únicos de los humanos, cuyo tratamiento representa un reto en donde existen pocas opciones con buenos resultados.Objetivo: Evaluar el uso de verapamilo y las prendas de compresión en pacientes con cicatrización patológicacomo consecuencia de quemadura. material y método: Incluimos pacientes con cicatrización patológica,ya sea queloide o hipertrófica, causada por quemadura que mencionaron no haber tenido beneficio con el usode prendas de compresión. La cicatriz fue evaluada con fotografías seriadas, escala de Vancouver y Posas.Los resultados fueron comparados con la prueba de t de Student. Resultados: Incluimos 13 cicatrices en 11pacientes. La localización de las cicatrices fue en brazos 4, piernas 4, cara y cuello 1, y abdomen 1. La dosisde verapamilo se calculó a 0,03 mg por kg. Las inyecciones se aplicaron intralesionales y se administraroncada 7 a 10 días hasta completar 6 sesiones. Encontramos mejoría en los siguientes parámetros de la escalade Posas: pigmentación (0,01), pliabilidad (0,01), endurecimiento o grosor (0,005), prurito (0,003) e irregular (0,004). En la escala de Vancouver elevación (0,008), pigmentación (0,014), vascularidad (0,022) yflexibilidad (0,014). No encontramos efectos adversos con la administración de verapamilo. Conclusión: Elverapamilo fue útil en conjunto con las prendas de compresión para mejorar las condiciones de la cicatrizqueloide e hipertrófica causadas por lesiones por quemadura.

ABSTRACT

Scar formation is a consequence of the wound healing process that occurs when body tissues are damaged by a physical injury. Hypertrophic scars and keloids are pathological scars resulting from abnormal responses to trauma and can be itchy and painful, causing serious functional and cosmetic disability. The current review will focus on the definition of hypertrophic scars, distinguishing them from keloids and on the various methods for treating hypertrophic scarring that have been described in the literature, including treatments with clearly proven efficiency and therapies with doubtful benefits. Numerous methods have been described for the treatment of abnormal scars, but to date, the optimal treatment method has not been established. This review will explore the differences between different types of nonsurgical management of hypertrophic scars, focusing on the indications, uses, mechanisms of action, associations and efficacies of the following therapies: silicone, pressure garments, onion extract, intralesional corticoid injections and bleomycin. .

ABSTRACT

OBJETIVO: O objetivo do projeto de pesquisa é determinar a incidência das complicações intra e pós-operatória da dacriocistorrinostomia externa efetuadas pela autora, no Hospital São Paulo, no período de 10 de abril de 2009 a 15 de janeiro de 2010. MÉTODOS: Estudo retrospectivo observacional dos prontuários dos pacientes submetidos à dacriocistorrinostomia externa no Hospital São Paulo, no período de 10 de abril de 2009 a 15 de janeiro de 2010.Os pacientes foram submetidos à avaliação oftalmológica: inspeção ocular, biomicroscopia,Teste de Milder, Teste de Jones I e II e exame de dacriocistografia. É mister salientar que os pacientes incluídos neste estudo foram examinados pela autora deste trabalho. RESULTADOS: Neste trabalho, verificou-se que das 15 dacriocistorrinostomias externas realizadas as complicações intra-operatórias encontradas foram: sangramento(46,66 por cento), perda do retalho da mucosa nasal (26,66 por cento), utilização do martelo na osteotomia(6,66 por cento). Em relação as complicações pós-operatórias citamos: fístula cutânea (6,66 por cento), cicatriz hipertrófica (6,66 por cento). Observou-se 1 caso de falência, correspondendo a 6,66 por cento das cirurgias realizadas. O tempo médio de acompanhamento pós-operatório dos pacientes foi de 4 meses. CONCLUSÃO: Neste trabalho, o índice de sucesso obtido por esta técnica cirúrgica foi grande, podendo afirmar que a dacriocistorrinostomia externa oferece ótimos resultados.


PURPOSE: The research project goal is to determine the incidence of complications during the external dacryocystorhinostomy and post surgery done by the author, in São Paulo Hospital, from april 10th , 2009 to january 15th, 2010. METHODS: This is a retrospective study of the records of the patients submitted to external dacryocystorhinostomy in São Paulo Hospital, from april 10th 2009 to january 15th, 2010.The patient were submitted to the ophtahalmologic evaluation: Ocular inspection, biomicroscopiy, Milder Test, Test of Jones I and II and dacryocystograph exam. It is important point out, that the included patients in this study, the patients were examined by the author of this work. RESULTS: This study was verified that out of 15 external dacryocystorhinostomy done, the complications were: bleeding (46,66 percent), oddment loss of the nasal mucosa (26,66 percent,utilização) of the hammer in the osteotomy (6,66 percent).The pos-operatory complications: cutaneous fistula (6,66 percent), hypertrophic (6,66 percent). Among the 15 surgeries there was only 1 failure corresponding to the 6,66 percent. The average time of surgery follow-up was of 4 months. CONCLUSION: The external dacryocystorhinostomy offers great results,according this study.

ABSTRACT

The formation of hypertrophic scars is common following healing of the burn wound, particularly in children. The face is one of the areas of the body most frequently affected by burns. Scar formation as a result of burn wounds leads to contraction of the formed granulation tissue, which causes both aesthetic and functional impairment for the patient. Scarring has major psychological and physical repercussions. Scarring on the face and visible regions of the body can be very distressing for the patient. Prevention of scars involves early and continuous use of a compressive orthesis. However, their efficacy is often limited to the facial region because of the contours of this area of body. This paper describes a clinical case of post-burn hypertrophic scars treated with silicone gel sheeting applied with pressure under custom made auto-polymerizing resin stent.

ABSTRACT

A cicatriz de cesariana como causa de morbidades ginecológicas, algo até pouco tempo ignorado pelos ginecologistas, é atualmente foco de importante discussão. O aumento atual da via abdominal para o parto faz desse tópico um assunto de extrema importância no cotidiano médico. O sangramento pós-menstrual sem causa aparente tem, na maioria das vezes, a cicatriz de cesariana como fator causal. Pode ocorrer, também, o comprometimento da fertilidade ou pela dificuldade de implantação embrionária ou pela piora da qualidade do muco cervical, em razão do sangramento uterino anormal. Discute-se, hoje em dia, qual o melhor método para o diagnóstico das alterações uterinas decorrentes da cicatriz de cesárea. A investigação pode ser feita inicialmente pela utrassonografia transvaginal, mas a histeroscopia tem maior acurácia além de permitir o tratamento específico. Em todas as situações, a investigação e o bom planejamento terapêutico só podem ser feitos quando o ginecologista conhece bem essa afecção.


The cesarean scar as cause of gynecological morbity, fact that used to be ignored by gynecologists, is the subject of an important discussion nowadays. The current increase in the number of abdominal delivery makes this subject a constant in our daily pratice. The post-menstrual uterine bleeding without an apparent cause has, in the majority of cases, the cesarean scar as only predisponent factor. There might also be infertility due to the difficulty of embryonic implantation or to the presence of the blood making the cervical mucus hostile. Currently, there is a discussion on the best method for the diagnosis of uterine alterations due to cesarean scar. The investigation may be initially accomplished by means of transvaginal ultrasound, but only the hysteroscopy may be precise in the diagnosis and allow specific treatment. In all situations, the investigation and adequate therapeutic planning can only be made if the gynecologist has ample knowledge on this pathology.

ABSTRACT

PURPOSE: Hypertrophic scarring and depression are the principal problems of burn rehabilitation. This study was done to verify the effects of skin rehabilitation massage therapy (SRMT) on pruritus, skin status, and depression for Korean burn survivors. METHODS: A pretest-posttest design using a nonequivalent control group was applied to examine the effects of SRMT for 3 months in a group of 18 burn survivors. The major dependent variables-including pruritus, objective and subjective scar status, and depression-were measured at the beginning and at the end of the therapy to examine the effects of SRMT. RESULTS: Burn survivors receiving SRMT showed reduced pruritus, improved skin status, and depression. The remaining scar also showed improvement in skin pigmentation, pliability, vascularity, and height (compared to the surrounding skin) as measured on the Vancouver Scar Scale (VSS). CONCLUSIONS: The findings demonstrate that SRMT for burn survivors may improve their scars both objectively and subjectively, and also reduce pruritus and depression.
